The Brookside access and continuity model
Using patient segmentation (created by Connected Care using the John Hopkins Adapted Clinical Groups) and Anima online consultation software the team have enabled the use of Continuity Teams (Patient Health Teams Model). They implemented a RAG rating for the problem and patient within our integrated clinical triage dashboard, a UK first, facilitating efficient non-clinician triage. Our Continuity Teams model of care splits patients into usual teams including a Partner, Salaried GP, Physician’s Associate, Social Prescriber and Patient Services Liaison (member of our reception team). Using these tools, we have improved access, continuity, reduced clinician admin and made best use of ARRS roles. Prior to go live, the team completed extensive research into the online consultation software available and worked closely with the Connected Care and Anima teams to enable smooth integration with the clinical system. This work has not only impacted Brookside Group Practice but has had clear wider system impact. The team have led the adopting of segmentation across Berkshire West as well as presenting at the ICB Digital Regional Summit, Best Practice London, an educational session for all Berkshire West GPs and aided various practices and PCNs to implement similar models. The team have won the digital HSJ award for innovation in primary care recently.
