Long Term Conditions: supporting primary care to optimise clinical care, remote care and self care post Covid. The UCLP Proactive Care Frameworks
The UCLPartners Proactive Care Frameworks are a major innovation to support long term condition management in post-COVID primary care. They have been developed by GPs and pharmacists for 6 conditions: atrial fibrillation, hypertension, cholesterol, diabetes, COPD and asthma. They are being used by PCNs across England. The frameworks include search tools built for EMIS and SystmOne that risk stratify and prioritise patients, pathways to optimise remote care and self-care, optimal use of the wider workforce to personalise care and reduce clinician workload, plus digital and other resources. The UCLP frameworks have now been adopted into a new national programme called NHS Proactive Care @Home.
