Pioneering Population Management in our changing times: a case study in supporting C&YP after Covid

PCN Transformation Theatre

Central Cheltenham PCN was amongst three PCNs in the country to embark on Wave 2 PHM program in October 2019. The intense 20-week program has given us the skills, knowledge and experience of using PHM approach to identify our cohort selection and design an evidence-based intervention addressing wider determinants of health. We continued our PHM work with a new cohort of Children and Young People (CYP) to maximise the opportunities for early intervention and reduce the negative impact of the pandemic on the emotional and physical wellbeing of our local population. Our key assets includes a new role of a dedicated Social Prescriber Link Worker for CYP and our project stakeholders including the Cheltenham Borough Council, the local Community Wellbeing Service, Schools, Trailblazers (school-based mental health services), CCG Commissioners of Children and Young People Mental Health Services, Business Intelligence team, CCG Safeguarding Lead amongst others.

  • - Identify the key barriers and enablers to a PHM approach at a PCN level using Children and Young people cohort as an example
  • - Understand the importance of context and relationship building alongside robust linked data sets
  • - Recognise the application of PHM approach in the forthcoming PCN DESs
