Learning from the PHM development programme: so much more than the Emperor's new clothes
PHM has gained popularity in NHS policy and practice. This session will outline how the principles can be used to improve population health at ICS, ICP, Place and Neighbourhood levels. Examples of practical applications of PHM to addressing real challenges will be covered as well as reflection on some of the challenges but also the opportunities that PHM provides.
