Optimising your stoma care pathways in primary careto be confirmed

The StoMap Programme Baseline Report of 2019 highlighted that up to 85% of ostomates have experienced common problems such as skin irritation, blockage, leakage, difficulty in attaching and removing appliances, but many ostomates do not seek professional help. This consequently leads to escalating healthcare costs due to increased product use and product wastage and unnecessary referrals to specialist stoma services. Further evidence of the need for change in stoma management includes the lack of scrutiny around how stoma product procurement is monitored, non-adherence to NHS conflict of interest policy, and variation in product use. In this session, you will hear from Soar Beyond Medicines Optimisation and Senior PCN Clinical Pharmacists about the opportunities to support improved stoma management using the Soar Beyond PQRS (Patient, Quality, Risks, Savings) approach.
