In this presentation Mohammed reflects on his 23 years of experience in delivering medicines optimisation services to NHS GP practices across the country and he shares his insights on how small changes can deliver considerable outcomes in patient care. He will share real world examples of how he and his team of clinical pharmacists have not only delivered millions of pounds of prescribing efficiency savings, but also tangible reductions in secondary care admissions and reduced GP appointments through effective use of GP practice pharmacists, technicians, and, nurses. He will summarise what he looks for in prescribing/medicines management audits when he is supporting the CQC inspection team at GP practices, and, how GP practices can make use of MEG services to not only improve patient care, but also to clearly demonstrate audit outcomes and improvements.
