In this session we will look at the challenges facing PCNs to deliver effective Population Health Management, to realise the potential of Additional Roles, to work collaboratively across healthcare systems in an unprecedented way and to rise the challenge of unprecedented demand on primary care.

Technology is an enabler but are the solutions available to us in GP IT Friend or Foe? Do these solutions meet the challenges of today, and more importantly, are they on course to meet the ambitions of PCNs and primary care in the future?

We will explore, through an interactive discussion which draws on experts from primary care and technology, what solutions must do to help PCNs understand their population, how technology can support redesign of care around the needs of patients, where technology can equip the primary care workforce to work efficiently and to work at scale across health economies.
