managed and the potential impact it is having on the healthcare system.

Addressing chronic disease early is key to managing and eventually reducing the burden on the NHS. For this reason it is at the forefront of the ICB agenda.

However, with workforce shortages and multiple ever-changing targets, this task is often difficult to coordinate for many primary care clinicians.
