Hounslow Consortium GP Federation is working in partnership with Hounslow and Richmond Community Trust to dissolve the boundaries of traditionally separate 'primary' and 'community' services by embedding their Federation, and additional roles teams in the Trust. These staff all work in Primary Care and for the Practices however are able to access the back office support from the Trust alongside professional support and supervision for the clinicians within the team. Hounslow's Director of Primary Care and the Clinical Director for Feltham and Bedfont Primary Care Network will explore the main components of the model, what has been a sucess and links to the recommendations set out in the Fuller Stocktake.
