What a dietitian can do in your PCN?

Adding a dietitian to the PCN team can enhance chronic disease management and preventative health through nutrition care. A dietitian in a PCN provides nutrition assessments, counseling, and education to patients for conditions like diabetes, high blood pressure, high cholesterol, obesity, gastrointestinal issues, malnutrition, food allergies, intolerances, and more. They work collaboratively with the patient and their primary care provider to set nutrition goals, empower patients, recommend supplements if needed, and monitor progress. Having access to a dietitian as part of a PCN allows patients to receive individualised nutrition care and support. This facilitates better health outcomes and supports the PCN in meeting DES, QOF and care targets.
