From April 2025, practices registering significant numbers of patients outside their catchment will need commissioner approval before expanding further. The move, set out in new GP contract guidance, reflects growing use of digital-first models that make it easier for patients to register remotely.
Integrated Care Boards will decide when approval is required, setting thresholds locally. Their focus will be on whether practices can maintain safe access to services, sustain their workforce, and continue delivering core care such as screening, vaccinations, and chronic disease management. Extra scrutiny is expected where practices grow quickly or where patients need regular in-person appointments.
The guidance makes clear that innovation in registration models must be balanced with measures that protect continuity and safety. Sustainable growth will depend on practices and commissioners working hand in hand.
