Early onset type 2 diabetes (EOT2) is an increasingly common and aggressive condition, carrying a higher risk of complications, disproportionate years of life lost, and significant inequalities in outcomes. This presentation will explore the unique risk profile of EOT2, the impact of social determinants of health, and the challenges faced in delivering effective primary care to this cohort. Drawing on practical tools, evidence-based interventions, and real-world experience, we will focus on embedding excellence in everyday practice. From accurate diagnosis and proactive recall systems, to medication optimisation, team-based care, and meaningful lifestyle support. We will also discuss the importance of language in consultations, and how stigma-free, empowering communication can transform engagement and outcomes.
