Key responsibilities.

'

Clinical assessment and management.

  • Conduct comprehensive assessments of patients presenting with acute and chronic conditions, utilising advanced clinical reasoning skills.
  • Initiate and implement appropriate treatment plans, including medication administration, wound care, and minor injury management.
  • Provide expert advice and guidance on urgent and emergency care to patients and colleagues.

Patient care and education.

  • Deliver compassionate and high-quality care, ensuring patients receive timely and effective treatment.
  • Educate patients and their families about health conditions, self-care strategies, and preventive measures.

Emergency response.

  • Respond rapidly and effectively to medical emergencies within the primary care , providing immediate life-saving interventions.
  • Coordinate with emergency services and facilitate smooth transitions for patients requiring further care.

Chronic disease management.

  • Support patients with chronic conditions, such as asthma, diabetes, and heart disease, by conducting regular reviews and optimizing treatment plans.
  • Collaborate with the multidisciplinary team to ensure holistic and integrated care.

Community engagement.

  • Participate in public health initiatives, including vaccination programmes and health education campaigns.
  • Engage with local communities to promote health awareness and address health disparities.

Team collaboration.

  • Work closely with GPs, nurses, healthcare assistants, and other allied health professionals to deliver coordinated and comprehensive care.
  • Contribute to case discussions, clinical audits, and quality improvement projects.

Documentation and compliance.

  • Maintain accurate and detailed patient records using electronic health systems (e.g., EMIS or SystmOne).
  • Ensure compliance with NHS policies, CQC standards, and GDPR regulations.

  • Pursue ongoing professional development and training to enhance clinical skills and stay abreast of the latest advancements in primary care.

Qualifications & experience.
