Job summary.

  • Conduct advanced assessments, diagnostics, and management of patients with complex and varied health conditions.
  • Utilise advanced clinical skills to develop and implement comprehensive care plans, ensuring optimal patient outcomes.

 

Patient care excellence.

  • Provide high-level care, including advanced diagnostic and therapeutic interventions, and manage complex cases independently.
  • Offer expert advice and support to patients, families, and the wider healthcare team, ensuring a holistic approach to care.

 

Innovation and improvement.

  • Lead and participate in quality improvement initiatives, implementing evidence-based practices to enhance service delivery and patient care.
  • Drive the development of new clinical protocols and procedures, contributing to the advancement of primary care services.

 

Team collaboration.

  • Collaborate closely with GPs, nurses, and other healthcare professionals, fostering a team-based approach to patient care.
  • Engage in multidisciplinary team meetings and strategic planning to ensure integrated and effective care delivery.

 

Education and mentoring.

  • Mentor and support junior nursing staff and colleagues, providing guidance and sharing expertise to promote professional development.
  • Engage in continuous professional development, staying updated with the latest clinical advancements and best practices.

 

Compliance and governance.

  • Ensure all clinical activities comply with NHS policies, care quality commission (CQC) standards, and general data protection regulation (GDPR) requirements.
  • Maintain accurate and up-to-date patient records, ensuring confidentiality and adherence to regulatory standards.

 


 

Person specification.

 

Qualifications & experience.

·        Advanced nurse practitioner qualification or equivalent.

·        Master’s degree in advanced practice or a relevant clinical field.

·        Registration with the nursing and midwifery council (NMC) as a nurse practitioner.

 

Skills and competencies.

·        Advanced clinical assessment, diagnostic, and treatment skills.

·        Exceptional clinical judgement and decision-making capabilities.

·        Proficient in using electronic health record systems for documentation and care coordination

 

Knowledge.

·        In-depth knowledge of advanced nursing practice, including current best practices and emerging trends in primary care.

·        Comprehensive understanding of NHS policies, clinical guidelines, and legal and ethical frameworks governing healthcare delivery.

·        Awareness of public health issues, chronic disease management, and strategies for improving patient care outcomes.

·        Familiarity with quality improvement methodologies and their application in clinical settings.

·        Understanding of GDPR regulations and their implications for patient care and record-keeping.
