Key responsibilities.
Medication management.
- Conduct comprehensive medication reviews,
assessing the efficacy, safety, and appropriateness of prescribed
therapies for individual patients.
- Develop and implement medication management
plans, including recommendations for drug therapy adjustments and
monitoring for adverse effects.
Patient care excellence.
- Provide expert advice on the selection, dosage,
and administration of medications, ensuring optimal therapeutic outcomes.
- Educate patients and caregivers on medication
use, potential side effects, and adherence strategies to enhance patient
understanding and compliance.
Clinical support.
- Collaborate with GPs, nurses, and other
healthcare professionals to integrate pharmaceutical care into patient
treatment plans and care pathways.
- Participate in clinical decision-making
processes, offering insights and recommendations based on clinical
evidence and patient-specific factors.
Quality improvement.
- Lead and participate in quality improvement
initiatives related to medication safety and efficacy, including audits,
protocol development, and implementation of best practices.
- Contribute to the development and review of
clinical guidelines and medication protocols, ensuring they align with
current evidence and regulatory standards.
Education and training.
- Provide training and support to healthcare staff
on medication management, pharmaceutical care practices, and new
treatments.

Compliance and documentation.
- Ensure all activities comply with NHS policies,
care quality commission (CQC) standards, and general data protection
regulation (GDPR) requirements.

Development
- Engage in research and contribute to clinical

Person Specification.
Qualifications & experience.

Master’s degree
in pharmacy (MPharm) or equivalent.
·
Registered with
the general pharmaceutical council (GPhC) or equivalent professional body.
·
Postgraduate
qualifications in clinical pharmacy or a related field are desirable.
·
Significant
post-registration experience in a clinical pharmacy setting, with proven
expertise in medication management and patient care.

Demonstrated
ability to work collaboratively within a multidisciplinary team and lead
quality improvement initiatives.
