Libby Dawe is a Project Manager at Health Innovation South West. She specialises in driving innovation to address health inequalities in primary care across Somerset, Devon, Cornwall and the Isles of Scilly, particularly in Cardiovascular Disease prevention. Libby is supporting the rollout of the Somerset Heart Failure Champions Programme, an initiative aimed at improving diagnosis and management of heart failure in primary care. Libby will be sharing early insights from this work, including the development of a sustainable community of practice and the impact of targeted interventions on patient outcomes.
