I have been working as a Mental health nurse for 27 years, within a range of secondary care teams including home treatment and community mental health teams. I moved to Primary care 2 1/2 years ago and set up a mental health and wellbeing team within the PCN, as an ARRS role employed by CWP/PCN. Since establishing the team we have developed a service that is valued and respected by GP, Secondary care providers, Patients and families I have supported with presenting the model we have in CHAW at local transformation events as i can demonstrate the positive impact we have made with our collaborative model approach with PCN ARRS roles.
