Tash is the Transformation Manager for Brookside Group Practice/Earley+ PCN. Brookside is a 30,000 patient practice with 4 sites and is an autonomous PCN. The Practice is forward thinking, digitally innovative and are often early adopters of new IT solutions. Tash has worked in the practice for over 5 years doing various roles but has recently moved into the role of Transformation Manager after being the Digital Transformation Manager for 2 years. Some of the projects she has led on include; setting up a non-clinician triage process, moving to Anima online consultation software, embedding patient segmentation and the use of a Patient Health Teams Model.
